That is why we came together to form MD Value Care (MDVC). We are an ACO made up of a group of local doctors from independent primary care and specialty practices in Richmond, VA who have joined together on a mission to provide better care to our patients. We are committed to helping each other navigate the changing healthcare landscape and achieve world class results in quality, patient experience, and cost-effectiveness.<\/p>\n

To ensure that our providers have the tools necessary for constant evaluation and improvement of patient care, our comprehensive Quality Management Program includes physician and staff education, hands-on training on the ACO 33 clinical metrics used in the Medicare Shared Savings Program, ACO 33 data abstraction and GPRO submission, CAHPS Patient Survey process, provider scorecards that include performance benchmarks for driving process improvement, and quarterly high risk patient reports. This program also includes data analysis to assist in enhancing practice clinical workflows that support quality metrics and improve patient care.<\/p>\n
